| dc.description.abstract | ABSTRACT This study discusses utilization of regional health insurance (Jamkesda) in the District of Kulon Progo with Alberch Hirshmans theory that i.e. exit, voice, and loyalty. These three approaches are used as a control form of factors affecting jamkesda utilization. Study of jamkesda utilization is regarded important because it gives an image of health service implementation after Universal Health Coverage application. Three main goals of UHC application are (1) equality in accessing health service, (2) good health service quality to improve the users, and (3) protection of financial fall risk. The District of Kulon Progo Jamkesda is started in 2009 with the publication of The District of Kulon Progo Regional Regulation Number 6 Year 2009 about Regional Health Insurance System. Jamkesda management in the District of Kulon Progo develops rapidly which can be observed when there is local leader transfer from Regent of Kulon Progo Toyo Dipo Santoso to Hasto Wardoyo that implication the emergence of Regent Regulation Number 65 Year 2011 about Jamkesda System Implementation Instruction. This regent regulation is the beginning of Universal Health Coverage application in the District of Kulon Progo. Three main questions in this research are (1) how the utilization of jamkesda programin the District of Kulon Progo in 2013 is, (2) how exit and voice characters in jamkesda program utilization are, and (3) how the strategies to maintain loyalty are. Type of research used is secondary data analysis. This research main source is Pemetaan Perilaku Komplain Peserta Jamkesda di Kabupaten Kulon Progo survey data done by Department of Public Policy and Management Faculty of Social and Political Sciences UGM lecturer team cooperating with TIFA Foundation. Data collecting technique is done by using documentation and interview technique. Data analysis technique done is descriptive analysis. Research results show that not all of people or respondents using jamkesda for their medication in 2013. Predisposing factor shows that jamkesda users people are people in productive age, low educational background, and working in non-formal sector. Enabling resources show people understanding toward jamkesda users right is still very low. Loyalty characteristic in utilizing jamkesda in the District of Kulon Progo is very dominant compared to exit and voice characters. Exit character is located at inpatient class and health service quality. Voice character, dissatisfied people deliver their complaint directly to the health facilities staffs. Loyalty character, people attachment arisen is categorized into two forms i.e. attachment based on satisfaction and dissatisfaction. Strategy to maintain users attachment is by two ways i.e. strengthening voice function and suppressing exit function. First, participation improvement of low and middle educated people by implicating public consultation and opening discretion space of service officers in the frontline should be done. Second, regional government and their networking must pay attention to total coverage in jamkesda implementations monitoring and evaluation. | |
